Probate Forms

 

Probate forms have been developed by the State of Wisconsin Records Management Committee (RMC).  All standard State forms used in the processing of probate cases are available from your County Register in Probate Office or at the Wisconsin State Supreme Court website. All standard State forms are available in two formats:

  • "pdf" format - print the form and complete by hand or by typewriter

  • "doc" format - Microsoft Word fillable form available on the internet

A Form Summary is also available for each form.

Some counties have additional "county specific forms" that have been developed because a standard State form does not exist.  Please contact your County Register in Probate to obtain the form or visit your local County website for more information.
